THIS POST IS AVAILABLE TO ALL APPLICANTS WHO CAN DEMONSTRATE RELEVANT PROFESSIONAL REGISTRATION, EDUCATION TO DEGREE LEVEL OR ABILITY TO DEMONSTRATE EQUIVALENT LEVEL OF KNOWLEDGE

An opportunity to join a well-established integrated, innovative and enthusiastic team committed to providing high quality services for predominantly older people who have mental health difficulties.

The successful applicant will be expected to continue to maintain and build upon the already positive working relationships with other AWP teams, Virgin care and voluntary and private organisations.

They will be expected to show a knowledge of person centred care, monitoring of new medications and ongoing assessment of a person’s mental health.

The successful post holder must also be committed to working with clients and carer's to establish flexible and creative care packages aimed at maintaining people’s independence within their own homes for as long as possible.

Good communication and organisational skills together with an enthusiasm and commitment to providing quality skills are essential along with the willingness to be active and supportive team member.

As part of the Community Mental Health Team you will work alongside the Therapy Team and Intensive Support Team thus enabling you to offer a dynamic and holistic service to users and their carers.
