Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) provides inpatient and community-based mental health care for people living in Bath and North East Somerset (B&NES), Bristol, North Somerset, South Gloucestershire, Swindon and Wiltshire. You will provide a defined drug and alcohol service to an allocated caseload of service users within the Specialist Prescribing Team, having responsibility for the day to day service users. You will also be responsible for the assessment of needs, the planning and implementation of treatment and care to meet those needs as agreed within the Multidisciplinary Team and/or Supervisor, co-ordinating care (with supervision) and contributing to the evaluation of care across a variety of settings where specialist treatment is available. Moreover you will take a lead role in delivery of alcohol detoxes under the supervision of the services clinical lead. 

Liaise as appropriate with other members of the Multidisciplinary Team, and other external agencies involved in the care if individual service users. 

Participate in the Integrated Governance process, particularly as they relate to own practice, contributing to the monitoring of service provision and performance and standards of care, through data collection and audit processes. 

Liaise with other services and agencies involved in the provision of services to people with drug and/or alcohol problems.

The successful applicant must suitably qualified e.g. BA (Hons) Nurse, experienced, skilled and hold a current registration; NMC.

 The post holder will be accountable to the Band 8 Service Manager, and will be based at Southcote Road, Bournemouth.
