Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) provides inpatient and community-based mental health care for people living in Bath and North East Somerset (B&NES), Bristol, North Somerset, South Gloucestershire, Swindon and Wiltshire. We also provide specialist services extending throughout the south west.

The poor physical health and life expectancy of individuals with mental health problems is deeply concerning. Therefore, AWP working with Bristol Mental Health are excited to be able to recruit to the post of Associate Practitioner for Physical Health and Wellbeing.
If your application was successful you would be working in a small team of unregistered Associate Practitioners led by a Registered Mental Health Nurse to run physical health clinics across four of our sites in Bristol and with individuals in their own homes.
Your role would be to engage individuals suffering from serious mental illness either in clinics or in their own homes to assess their physical health, to take blood samples, measure blood glucose, prepare and take ECG readings, assess smoking, alcohol, diet, exercise and to undertake brief interventions.
You would be based at Callington Road Hospital in Bristol alongside your team. However, you would be expected to be mobile so access to transport is essential for this role.
