Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) is a significant provider of high quality mental health services across a core catchment area covering Bath and North East Somerset (B&NES), Bristol, North Somerset, South Gloucestershire, Swindon and Wiltshire. The Trust also provides specialist services for a wider catchment extending throughout the south west.

Our aim is to enable and empower people to reach their potential and to live fulfilling lives through providing recovery and reablement focused services that yield positive outcomes for our service users and their carers.

Specifically, The Trust provides services for people with mental health needs, with needs relating to drug or alcohol dependency and mental health services for people with learning disabilities. We also provide secure mental health services and work with the criminal justice system.

Increasingly AWP provides treatment and care in people's own homes and other community settings, reflecting the preferences of our service users. Our community services are supported by high quality inpatient services that provide short term assessment, treatment and care.


 

An exciting opportunity has arisen in South Gloucestershire Primary Care Liaison Service.

This service affords rapid access to credible specialist mental health expertise for GPs, Primary Care staff and other referring agents who are requiring support and expert advice on meeting the mental health needs of the people within their caseloads.
Our service aims to ensure that patients in Primary Care have their mental health needs more clearly assessed, defined, cared for and treated by Primary Care Health Care Professionals and that the pathways, to best meet service users’ needs are optimised.

The mental health liaison practitioner will prioritize assessments based on clinical need and risk, making evidence-based expert recommendations for care and treatment within and across multiple care pathways, ranging from those within primary care and where necessary into secondary specialist mental health services.

The successful applicant will hold an appropriate professional registration. Have a strong proven clinical base and demonstrate clear clinical and leadership skills. The team will be based at the Civic Centre Kingswood Please note this post is also open to other clinical practitioners outside of nursing.Previous experience or an interest in old age psychiatry would be an advantage for this post.
