The successful applicant will be required to:

-work within the South Gloucestershire Youth Offending Service.

-be a link and a bridge between the Youth Offending Service, CAMHS, Social Care, Education, Voluntary sector and frontline/tier 1 services; providing referral guidance, signposting, consultation as well as face to face work with young people and their families.

- work in partnership with a range of primary health care professionals, statutory and non-statutory agencies and voluntary organisations in delivering a high quality, clinically effective health services to YOS service users.

- promote and improve the mental and psychosocial health of YOS service users. To supervise the assessment of care needs and the development, implementation and evaluation of appropriate packages of care, onward referrals and signposting. Take a role with training and development and data collection and service development.

- be able to work autonomously within an isolated role, develop and maintain links with professional group.
