We are looking for a highly motivated and enthusiastic person to join an innovative team working with women with primary care mental health needs. The purpose of this post is to support individuals access social opportunities (eg hobbies, volunteering, education, support groups) by conducting assessments and providing one to one support, advice, and signposting. The social prescriber will also refer to other organisations (eg debt advice, counselling, social care) if needed, and provide support to access these.
Experience of working with adults with mental health problems is essential, as are good written and interpersonal communication skills.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
