Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) provides inpatient and community-based mental health care for people living in Bath and North East Somerset (B&NES), Bristol, North Somerset, South Gloucestershire, Swindon and Wiltshire. We also provide specialist services extending throughout the south west.

We are the later life Complex Intervention and Treatment team for North Somerset. We are an integrated mental health team for older adults with functional mental health issues, and for those with a diagnosis of dementia.
We are seeking a qualified, experienced and passionate CPN to join us.
The role is full time, to be a care co-ordinator with a caseload of up to 25. You will be responsible for both health and social care.
We are a friendly team, which includes CPNs, social workers, OTs, support workers, social care workers, physiotherapist, psychologists, psychiatrists and admin.
There is an AWP team manager, job share North Somerset team managers and an acting AWP senior practitioner.
We are based in Windmill House in Kenn near Clevedon, close to the M5 junction.
Also based at Windmill House are North Somerset DEST (Dementia Enhanced Support Team) and North Somerset Memory Service.
You must have appropriate UK professional registration.
