Come and join the North Somerset Complex Intervention and Treatment Team,  based in Clevedon.  Our team is based just 5 mins from Jct 20 of the M5. We are a friendly supportive team who are passionate about improving the lives of older adult service users with complex mental health needs, including dementia.

In this integrated health and social care team we focus on the delivery of  specialist health and social care interventions, including carers support and supporting self-management.

As a Band 4 Support Worker in the team you will have a caseload of service users with a range of complex mental health needs, working within the CPA framework to deliver a range of health and social care interventions. You will be required to visit service users in their own homes, in care homes and at our community bases in North Somerset.

You can expect to receive excellent supervision and support and there will be opportunities for career development and training in order to support you in this role.
