 Community Mental Health Nurse -  Care Home Liaison Team.

An exciting opportunity has arisen for a Band 5, Registered Mental health Nurse to work in the Care Home Liaison team. This post offers varied and interesting work with older people who have severe mental health problems including challenging behaviour, living in residential and nursing care homes across South Gloucestershire. The post holder will work as part of a multi-disciplinary team to provide support to care homes, advice, short term interventions and training to support care home staff. Also to review residents in care homes that require a 117 review as required by health & social care. The service aims to prevent unnecessary hospital admissions and higher level placements wherever possible.

The post holder will work closely with mental health nurses, occupational therapist, psychologist and support workers in the care home liaison team. The team is part of the wider Later Life services in South Gloucestershire.

You will work flexibly across South Gloucestershire under the supervision of the team manager and the support of the senior practitioner.

Applicants should hold a professional registration, and preferably have experience in community work, working in care homes and working with older, vulnerable people.
