Band 6 CMHT clinician - Organic Pathway

Are you Dynamic? Motivated? Passionate about community based care? A registered Mental Health Practitioner?

If you can answer yes to these questions then we want you to apply for our band 6 organic pathway post within Sarum CMHT.

The post centres around care co-ordinating a complex caseload of service users who have organic diagnoses.  The role requires efficient multi-disciplinary working and taking the lead within the CPA process.  There is an expectation that you will be able to complete memory assessments as part of this role.
