We have a vacancy within our Dementia and Later Life Liaison Team based at the Victoria Centre in Swindon.

The service works to support older people with mental health illness, including dementia, who are admitted to the Great Western Hospital (GWH) and /or who live in care homes within Swindon.   Successful applicants will work predominantly in one of these two areas but with the expectation to work flexibly across the service

You will join a highly experienced and dedicated team who pride themselves on collaborative working with service users, carers and other professional, who provide assessment and treatment of mental illness. We are passionate about supporting care staff in the community to use person centred approaches to reduces distress and promote wellbeing of their residents. We also aim to minimise the need for hospital admission, however if admission is required we try to make this as short and smooth as possible.

You will hold an appropriate professional registration (NMC, HCPC etc) , will have a strong proven clinical knowledge within mental health and demonstrate ongoing professional development. Previous experience of working with older adults is desirable. The role will include use of computer systems so proficient use of IT is necessary.
