This is a fixed-term/secondment for 12 months. If you are an internal candidate on a substantive contract you may only apply for a secondment. Please ensure you have the support of your current line manager before applying.   At the end of your secondment you will return to your current post at your current grade.

This is a new post to work alongside our Swindon Borough Council colleagues with homeless people who have recently been homed. It is our aspiration to provide assessment of their mental health needs, and support them in access mental health services and support as needed.

The Swindon Primary Care Liaison Service are incredibly proud of the high quality service we offer our service users and carers.

We are a fast paced service who works flexibly and collaboratively to meet the needs of people with mental health problems in Swindon. We are the single point of access to mental health services and offer support to a range of primary health and social care providers to ensure that all service users that are referred to us are aligned to the most appropriate care pathway.

We are looking for an experienced mental health nurse, social worker or OT with an interest in mental health assessment and liaison. You will demonstrate post-registration experience at Band 5 or above and be able to provide evidence of continued professional development. Successful applicants will join a highly skilled team of professionals, offering robust and timely triage, face to face assessments and liaison with primary and secondary services. You will work with an ageless client group presenting with a wide range of mental health problems.

The service operates Monday – Friday 8am -6pm (excluding bank holidays).
