We welcome applicants from suitably qualified mental health professionals, RMN OT SW for the post of team manager of the Wiltshire care home liaison team.

This is a full time post working across Wiltshire.

The Wiltshire care home liaison team is responsible for providing assessments and support to individuals with a diagnosis of dementia who reside in a Wiltshire care home. The team work with GPs and care home staff  improving mental health recognition and treatment and avoiding hospital admission when possible.
